Geriatric Care Managers are experts in the field of aging. They help older adults, their families, and their care teams negotiate the challenges of aging, make informed decisions, and offer support. Geriatric Care Managers are responsible for creating individualized care plans for each client while keeping their lifestyle, their agency, and personal goals in mind. They often act in the capacity of liaison or patient advocate between the clinical team and the client's family. Geriatric Care Managers provide ongoing assessment, support, and monitoring.
Geriatric Care Managers work with a senior’s entire care team, which often includes family members, professional caregivers, therapists, nutritionists, social workers, elder care attorneys and other professionals. Additionally, Geriatric Care Managers are knowledgeable about resources, housing, and the associated costs in the community.
